Name change when newly divorced

Heloo I am newly divorced and my MSA states that my married name revert to my premarried name. Besides my MSA, the Entry of Judgment of Dissolution and SS card, are there any other legal documents I need? And how long does the process take? Can I make an appt at the Oceanside SS office?

To change the name on your Social Security card :

Step 1: Complete an Application For A Social Security Card (Form SS-5);

Step 2: Show us proof of your:
* Legal name;
* Identity; and
* U.S. citizenship (if you have not previously established your citizenship with us) or immigration status.

Step 3: Take (or mail) your completed application and documents to your local Social Security office.

Documents Social Security may accept to prove a legal name change include:

  • Marriage document;

  • Divorce decree;
  • Certificate of Naturalization showing a new name; or
  • Court order for a name change.

All documents you need for a Social Security card must be either originals or copies certified by the issuing agency.
