Minor name change

I got married last year. I have my daughter's updated birth certificate with her new last name. My question is-when I go to apply for a new card for her, will just her birth certificate be enough to get the change, or do I need to bring another form of identity?

Also, I attempted to call my local office in Somerset, ky, using the phone number provided by this website to ask them, and the number is disconnected. I'm not sure who to contact about this.

To change a name on a Social Security card you need evidence of:

  1. Legal name change
  2. Identity
  3. US citizenship or immigration status

Looks like your have (1), and probably (3) if your child is born in the US. If you have a passport this will meet the citizenship and identity requirements.

If you don't have a passport, the identity requirement can be met by a certified school record or transcript (current year) or similar.
