Replace Social Security Card for Child Under 12

This is to replace an child's Social Security card. The procedure is a bit different to replace an adult's Social Security card.

If you need to replace a lost or stolen Social Security card, you must visit a Social Security office or card center, or you must mail in necessary documents.

To obtain a replacement Social Security card :

Step 1: Gather documents proving your:

  • Child's identity; and

Social Security Card Center for Certain Cities

There are six cities which have "Card Centers" :

  • Las Vegas, NV
  • Minneapolis, MN
  • Orlando, FL
  • New York, NY (Brooklyn, Queens, the Bronx and Manhattan)
  • Phoenix, AZ (north Phoenix and downtown Phoenix)
  • Sacramento, CA

If you live in one of these cities, you may need to go to a 'card center' instead of a 'field office'. You must visit a card center if you need a Social Security card, your personal information needs updating, or your employer asked you to visit a Social Security office.
