Disability Analyst won't return my calls?

I have been leaving messages, with the required info included as directed by his voicemail, yet no returned call. I began leaving messages asking for a call back 5 business days, and a total of seven days ago. When should I expect a call back? How do I find out my analyst supervisors name and contact info to record a complaint etc?
