I am trying to get a copy of my birth certificate and it says I was a unnamed child and I have to have this form to get my birth certificate and I do not know how to get it. Could someone help me. I guess my parents did not name me until after they left the hospital. Thanks Connie

I'm not familiar with DD-214. There is a Department of Defense form DD-214, Certificate of Release or Discharge from Active Duty, but this doesn't match your situation.
