I am in need of a printout of the jobs I have worked WITH THE NAME OF THE EMPLOYER since 1976.
I have gone to my local SS office 4 times. I sit for hours NO ONE even talks with me I never get called. They close at 3pm.
Is there a form online that I can fill out to get this information

Obtaining detailed list of employers
You may obtain free of charge "Your Social Security Statement". This will show your earnings record, but not a list of employers. Here is a sample statement :

For detailed list of employers, you should use form SSA-7050-F4, Request For Social Security Earnings Information.
You should use form SSA-7050-F4 if you need:
1. Certified/non-certified detailed earnings information, which would include periods of employment or self-employment and the names and addresses of employers, or
2. Certified yearly totals of earnings, which would include total earnings for each year but not the names and addresses of employers.
