I got married in Jamaica last September. I would like to do my name change by mail because our social security office is very busy here. What exactly do I need to send in beside the form and my marriage certificate? Would my passport be enough?
Social Security name change through the mail
Yes, you may mail your completed application (SS-5) and documents to your local Social Security office.
All documents must be either originals or certified copies by the issuing agency. The SSA cannot accept photocopies or notarized copies of documents. Any documents you mail to the SSA will be returned to you along with a receipt.
