why would my x-employer want me to sign a paper
on this paper it says that i the worker would not seek a refund in the present, past or future? from medicare or social security
Refund of Social Security taxes
It's not clear what the employer is trying to do. If you work for two employers in a year, and your total wages are greater than the Social Security earnings limit ($106,800 in 2011) you may have excess Social Security taxes withheld -- and be entitled to a refund.
However, the claiming the refund is up to you when you file your income taxes, and should not concern your employer.
