I am the rep payee for my niece and i received a check. I want to depsit it but when i went to the bank they said i needed a form from ss office so they could set up a special account.... i found standard form 1199a online... direct deposit sign-up form...
is this the form the bank can use to set up an account or do i need to go back into the ss office?
Form SF-1199A and representative payees
As a general rule, it is not necessary to get an SF 1199A when developing direct deposit. However, there are some instances when the SF 1199A is required.
Form SF-1199A must be completed and signed by the beneficiary and the financial institution in the following situations:
* When there is a question of assignment of benefits.
* When an individual or institution acting as representative payee for several beneficiaries requests payment into a common checking or savings account.
