533. What information should be in the Reconsideration Disability Report?
The "Reconsideration Disability Report" asks you for new information about your claim. It asks whether there has been any change in the severity of your illness or injury, new limitations, restrictions that your doctor(s) have given you since you filed your claim. You will also be asked for information about new doctors seen or hospitalizations since you filed your claim, and new agencies (such as VA or vocational rehabilitation) you have seen. The form also asks for information about any changes in your daily activities, and a description of how your impairments affect your ability to care for your personal needs.
