I got married 5 1/2 years ago and decided to keep my madian name. Recently I have decided to change my name to receive my married name. Is there an easy way to change my name? I heard something about taking my marriage certificate, my ss card, and my id to the ss office and there was an easy way to change it. Is that true? or is there more to it?
Changing name on Social Security card
Yes, what you listed is essentially correct. Here are the steps :
Step 1: Complete an Application For A Social Security Card (Form SS-5)
Step 2: Show SSA proof of your:
* Legal name change
* U.S. citizenship (if you have not previously established your citizenship with SSA) or immigration status, and
* Identity
Step 3: Take or mail your completed application and documents to any Social Security office. Any documents you mail to us will be returned to you.
