Forms for Changing your Name

Just wonderign why I have be unable to change my name even though I have submitted the application - nearly 2 years ago and have gone to the social security office where I first submitted the application and they continue to tell me it will be done that day, even told me it was THEIR fault it had not been done after all of these months, and now I moved, I went to another office, they contacted the first office and I was to get a call when they received the paperwork for my name change, which included all certified copies of documents I provided 2 years ago to expedite the process. I have gone to SS offices at least 15-20 times in 2 years, made num,erous calls to them and yet, nothing is done. Any answers for me????
