While working on my Social Security payment application the local SS office t changed to my maiden name to my married name. The employees working on my application did this, as in my original application it is correct and every official paper I have gotten to this time has my correct maiden name. I receive a call by the local SS office that I needed to bring in my marriage license. I was married 42 years ago, and only have a "pretty" marriage certificate as described by the SS clerk. I was told I would have to bring in the official license. To order this, it cost $33.00. Why could they not look at my "pretty" license as it is the only one we have and it was the local SS office's problem?
Evidence required for name change due to marriage
The following quote from the SSA's Program Operations Manual may be helpful :
Marriage certificates, souvenir certificates, and certified copies of marriage records are considered marriage documents and acceptable evidence of a name change.
