Received letter from bank "Backup Withholding Warning". Told to contact SSA for Social Security Number Printout. What do I need to do to get this printout. This is for my Mom who is unable to travel to the office and when I tried to get the printout for her, I was told my Mom had to request it. I have tried to call several times (West Nyack, NY office) but there is no answer. What can I do now?

Backup withholding and "Social Security Number Printout"
It's hard to say what the issue is, but it's possible the bank is requesting a copy of the "Numident" to verify the Social Security number provided to the bank is correct. Please check back with the bank to see what they want. Also verify the bank has the correct Social Security number for the account.
The Numident is an electronic record of the information contained on an individual's original application for an SSN and subsequent applications for replacement cards. Numident printouts are not issued by SSA field offices. To obtain a Numident printout, an individual must send a written request to SSA's Central Office and pay a $16 fee.
