Where I can get a form SSA-L676. I could not find this form on Social Secutity Form online. I really don't have an idea how to get it; I have been searching on online sites. Please help me!! Thanks

Form SSA-L676 - Refusal to Process SSN Application
Form SSA-L676 appears to be a document for internal use by the SSA. It appears to be the form which is provided to an applicant when the applicant can not be provided a Social Security number.

Additionally, use the SSA-L676 when you are still unable to process an SSN application after the applicant previously received an SSA-L669 or SSA-L670 requesting evidence or an interview (see RM 00299.005 and RM 00299.010).
