Signature on Form SS-5 for Name Change

I am filling out a Form SS-5 in order to change my last name due to marriage. On line 17, "Your Signature", am I supposed to sign with my new name or my old name? I don't want to sign it wrong and then have to fill out the form all over again.

From "POMS Section : RM 10205.180 - Applicant’s Signature on a Form SS-5 Received in the Mail", you should sign with your new name (the name you want on the new card) :

If the (applicant) applies on his or her own behalf, the signature in item 17 must agree with the Name to be Shown on Card in item 1. For acceptable discrepancies, see RM 10205.180E.

However, even if you sign with an your maiden name, that is an acceptable discrepancy :

Acceptable signature discrepancies

When the number holder applies on his or her own behalf, the following discrepancies between the name in item 1 and the name in the signature block are acceptable:

  • The signature does not include the middle name.
  • The signature does not include the suffix.
  • A bride enters her new name in line 1 of item 1, but signs the signature block using her prior name.
  • The applicant signs with an obvious nickname, abbreviation, or initials.
