Change my last name to my mothers maiden name?

I want to change my last name to my mothers maiden name I am now 21. My father hasn't been in my life for the past 8 years, and he has married into another family and changed his last name to his wives. I want to change mine but how do I get this done? I live in Sterling, Co.
